We use a 12-week evidence-based hypertension management program designed to help you take ownership of your cardiovascular health.
The program promotes regular blood pressure self-monitoring at home using proper techniques, individualized support and hypertension education.
You will be provided with a free blood pressure cuff and our lifestyle training book when you begin the program.
Self monitoring makes a difference
Self-Monitoring Blood Pressure (SMBP) involves a patient’s regular use of personal blood pressure monitoring devices to measure and record blood pressure points in time outside of a clinical, community or public setting, typically at home.
- The American Heart Association recommends home monitoring for all people with high blood pressure to help the healthcare provider determine whether treatments are working.
- Charting and tracking your blood pressure improves blood pressure management.
- SMBP has proven useful in reducing the risk of death and disability with hypertension.
Important information about SMBP
- It’s important to take the readings at the same time each day, such as morning and evening.
- It is best to take the readings daily; ideally beginning 2 weeks after a change in treatment and during the week before your next appointment.
- Take multiple readings and record the results. Record the results using a BP log or the hypertension app HYPE.
- Your monitor also has built-in memory to store your readings, so please take it with you to your appointments.
