We use a 12-week evidence-based hypertension management program designed to help you take ownership of your cardiovascular health.
The program promotes regular blood pressure self-monitoring at home using proper techniques, individualized support and hypertension education.
You will be provided with a free blood pressure cuff and our lifestyle training book when you begin the program.
Self-Monitoring Blood Pressure (SMBP) involves a patient’s regular use of personal blood pressure monitoring devices to measure and record blood pressure points in time outside of a clinical, community or public setting, typically at home.
