All incoming students, regardless of classification, must complete the following consent forms for services at the Student Health Center and Pharmacy (SHCP).
For all first time visits to the SHCP, please complete the New Patient Form (Spanish Version).
For students seeking care, and for those have had previous appointments during the semester, please complete the All Visit Form. It is important to understand the All Visit forms must be submitted in for every visit.
Once you have completed the forms, please log into your MyBama page and under the student tab look for the Student Health Center and Pharmacy section to the right. Click on “access your Patient Portal” then choose messages + medical records + team and add an attachment to upload those forms directly to the SHCP.
Please note, your appointment time could be delayed if you have not completed and submitted the required forms. If you have trouble accessing your Patient Portal, you can email the completed forms to our Medical Records Department at shcmedicalrecords@ua.edu.
You may also upload you insurance information to the Patient Portal as well. Please remember you will receive a no-show fee of $50 for any appointment that is not canceled 24 hours in advance.
